Peak Achievement Trainer Rental AgreementAll you need to do to get started is to send a $1,100 deposit, plus $50 shipping and insurance charges, and the first two month’s rental of $450 each, totaling $2,050. We have to charge an appropriate deposit amount in order to protect our investment. There is a minimum of two months rental, which is all the time that most adults and children need to finish the early part of the training. We do recommend that after the early part of the training, you gradually decrease the frequency of sessions rather than stopping abruptly. This enhances transfer of training to academic and athletic tasks. This standard practice in neurotherapy allows you to monitor the user's continued course. Keeping the Trainer longer may also be advantageous if you wish to work with your doctor to decrease medication.To start, please sign and date the rental agreement, acknowledging that you have read the terms. For the initial payment, please send a check or money order. Once you have ordered the rental, it is non-cancelable and the first two months rental are not refundable. We will ship the Trainer to you in 10-14 days after receiving payment. Arrangements are made at the beginning of the rental period for payment of the subsequent months' rental by PayPal. Return shipping and insurance for the $5,995 cost to buy the Trainer is prepaid by the renter. Rental beyond 60 days is at $14 a day until the Trainer is received back in our office. The $1,100 deposit is returned to you upon receipt of the Trainer with all parts in like-new condition (excluding normal wear and tear usage), except for the Manual and Workbook. If you decide that you would like to purchase the Trainer, we will credit one-half of the rental fee towards the purchase price. The rental for the PBHT does not include the 5 DVDs and CDs that normally come with the purchased package. I agree to that the laws of the state of Kentucky will govern and to the jurisdiction of the Courts in Oldham County, Kentucky. I understand that normal wear and tear is included in the rental plan, but that I am responsible for any damage caused by abuse, mishandling, and/or loss.
